Flu Clinic

The annual flu shot clinic on campus will be on Monday, October 7th, from 9:00 am to 5:00 pm. It will be held in the Yehl ballroom. The most recent COVID vaccine will also be available.

Please print the form(s) below and bring them with you on the day of the event. The COVID consent form must be printed as as a two-sided page.
